ADÉNOCARCINOME LIEBERKHUNIEN PRIMITIF DE LA VESSIE CHEZ UNE FEMME DE 40 ANS : À PROPOS D’UN CAS

Résumé


Introduction : L’adénocarcinome liberkhunien primitif est une entité anatomoclinique rare des cancers de la vessie qui se développe sur une métaplasie glandulaire des cellules urothéliales de la vessie. Nous présentons un cas d’adénocarcinome lieberkhunien primitif de la vessie et discuterons la prise en charge thérapeutique, le pronostic et l’évolution de ce cancer.
Observation : Une patiente de 40 ans, multipare, a consulté pour hématurie terminale et pollakiurie. Devant la suspicion de tumeur de la vessie en échographie, une cystoscopie réalisée a mis en évidence une tumeur de la paroi antérieure et latérale gauche de la vessie. Les biopsies ont confirmé un adénocarcinome de type liberkhunien. L’imagerie par résonnance magnétique a montré une image hypodense de la paroi antérieure vésicale, rehaussée par le contraste avec infiltration du muscle pariétal. La coloscopie était normale. La patiente va bénéficier de 4 cures de chimiothérapie néoad juvante par FOLFOX6 avec une réponse clinique partielle.

ABSTRACT

Introduction: The primitive liberkhunian adenocarcinoma is a rare pathological entity of bladder cancer that develops in glandular metaplasia of urothelial cells of the bladder. We present a case primary lieberkhunian adenocarcinoma of the bladder and discuss the therapeutic management, prognosis and the development of this cancer.
Observation: A patient of 40 years, multipara, consults for terminal hematuria and pollakiura. To the suspicion of bladder tumor ultrasound, the cystoscopy performed showed a tumor of the anterior and left lateral bladder wall. Biopsies confirmed bladder liberkhunian adenocarcinoma. The magnetic resonance imaging showed a hypodense image of the anterior bladder wall, enhanced by the contrast with infiltration of the parietal muscle. Colonoscopy was normal. The patient will receive 4 cycles of neoadjuvant chemo therapy by FOLFOX6 with a partial clinical response. At the time of surgery by laparotomy, there is still a tumor occupying the left side of the bladder dome and infiltrating the parietal peritoneum and the left rectus muscle. Partial cystectomy, taking the parietal peritoneum, the round ligament of the left and some pellets left rectus muscle, is performed and completed by an ilio-obturator lymphadenectomy. In the aftermath of the surgery, the patient had an adjuvant to external beam radiation dose of 46 Gy. She was alive without recurrence after falling 17 months. Conclusion: The bladder adenocarcinoma is late discovery. The prognosis depends on the initial response to neoadjuvant therapy and surgical resection margins.
