PRISE EN CHARGE DES TRAUMATISMES FERMÉS DU REIN À PROPOS DE 35 CAS

Résumé


Objectif : Le but de ce travail était d’évaluer la prise en charge des traumatismes fermés du rein au sein de notre
établissement sanitaire.
Patient et méthodes : Il s’agit d’une étude rétrospective portant sur 35 cas de traumatisme fermé du rein, colligés au service d’urologie de l’hôpital principal de Dakar sur une période de 5ans étalée entre 2008 et 2013.
Résultats : Les patients se répartissaient en 27 hommes (90 %) et 8 femmes (10 %). L’âge moyen était de 27 ans.
Les étiologies étaient dominées par les accidents de la circulation (54 %). Le traumatisme atteignait surtout le côté droit (60%). La symptomatologie clinique était dominée par l’hématurie (93 %) et les lombalgies (90 %). Le  traumatisme était isolé dans 63 % des cas et associé dans le cadre d’un polytraumatisme dans 27%. Selon la classification de l’AAST retrouvait 25 grades mineurs I et II, 5 grades III, 2 grades IV et 3 grades V dont un décédé avant toute prise en charge. Tous les traumatismes mineurs ont bénéficié d’une surveillance clinique. Deux grades III ont bénéficié d’un drainage chirurgical de collection hématique. Un drainage de la voie excrétrice par sonde double j a été réalisée dans les grades IV. Une néphrectomie totale a été effectuée dans 2 cas des grades V. La mortalité était essentiellement représentée par le décès d’un patient présentant un grade V dans le cadre d’un polytraumatisme. La morbidité était  essentiellement à type d’urinomes persistants (10%), d’hématomes (15%) et d’infections(18).
Conclusion : Devant l’évolution favorable de nos patients ayant eu une simple surveillance et tenant compte des résultats de la littérature, nous pensons que le traitement conservateur est actuellement la méthode de choix en dehors de certaines indications ou la chirurgie s’impose.

ABSTRACT

Goal: The aim of this work was to improve the management of closed kidney in our health facility trauma.
Patients Et Methods: This is a retrospective study of 35 cases of blunt trauma, kidney collected at the urology department of the main hospital in Dakar for a period of 5 years spread between 2008 and 2013.
Results
: Patients were divided into 27 men (90%) and 8 women (10%). The average age was 27  years. The causes were dominated by traffic accidents (54%). The trauma reached above the right side (60%). The clinical symptomatology was dominated by hematuria (93%) and back pain (90%). The trauma was isolated in 63% of cases and associated under polytrauma in 27%. According to the classification of the AAST found 25 minors grades I and II, 5 grade III, IV grades 2 and 3 with a grade V died before taking office. All minor trauma underwent clinical monitoring. Two grade III underwent surgical drainage of hematic collection. Drainage of the urinary tract by double-ja probe was conducted in grades IV. Total nephrectomy was performed in 2 cases of grade V. Mortality was mainly represented by the death of a patient with grade V in a polytrauma. Morbidity was mainly type urinomes persistent (10%), bruising (15%) and infections (18).
Conclusion
:  Given the favorable evolution of our patients with a simple monitoring and taking into account the results of the literature, we believe that conservative treatment is currently the method of choice outside certain indications or surgery is required.
