FISTULES ENTERO-VESICALE ET VESICO-CUTANEE REVELANT UNE TUMEUR DE VESSIE CHEZ UNE FEMME

Résumé


Les fistules entéro-vésicales secondaires aux tumeurs de vessie sont rares, encore plus dans notre contexte où les tumeurs de vessie sont peu fréquents chez la femme.

Nous rapportons le cas d’une tumeur de vessie chez une femme révélée par une double fistule entéro-vésicale et vésico-cutanée.

Il s’agit d’une femme de 62 ans opérée par taille vésicale 5 ans auparavant pour une lithiase vésicale avec des suites opératoires simples. Sa symptomatologie actuelle s’est révélée 5 semaines avant son admission par un épisode d’hématurie totale caillotante avec une induration de la cicatrice opératoire, compliquée par la suite d’une abcédation puis d’une fistulisation avec notion de fécalurie. Différentes explorations nous ont permis d’objectiver la fistule et grâce notamment à la cystoscopie avec biopsie nous avons confirmé l’origine tumorale (carcinome urithélial). Une exploration chirurgicale a été indiquée mais non réalisée devant le refus de la patiente.

Les fistules entéro-vésicales sur tumeur de vessie sont rares et peu rapportées dans la littérature. Le diagnostic repose sur un faisceau d’arguments cliniques et paracliniques ; la cystoscopie garde une place importante. Le traitement repose actuellement sur la résection en un temps et en bloc des anses intestinales et de la vessie. Il n’y a pas de place au traitement médical ou palliatif sauf en cas d’une altération profonde de l’état général, ou du fait d’un processus néoplasique non résecable.
