ASPECTS EPIDEMIOLOGIQUES DES TUMEURS DE VESSIE DANS LE SERVICE D’UROLOGIE DU CHU POINT-G A PROPOS DE 185 CAS.

RESUME :

Introduction : Le cancer de la vessie est en fréquence, après le cancer de la prostate, le deuxième cancer de l’appareil urinaire. Il est nettement plus fréquent chez l’homme que chez la femme.

L’incidence du cancer de la vessie est à la hausse au Mali, devient un véritable problème de santé publique.

  • Chez l’Homme : 8,2/100 000 habitants et chez la Femme : 6,2/100 000 Habitants selon Registre cancers Mali 2015 [15].

 Notre travail s’inscrit dans ce cadre.

Patients et méthodes : étude rétrospective et descriptive colligeant 185 Patients hospitalisés dans le service d’urologie du CHU Point-G du 1er Janvier 2015 au 31 Décembre 2016, pour tumeur de vessie. Tous ces patients ont été retenus pour l’étude. Les aspects épidémiologiques (fréquence, âge, sexe, situation géographique, facteurs de risques), diagnostiques, thérapeutiques et évolutifs ont été analysés.

Résultats : 185 patients (102 Hommes et 83 Femmes)  présentaient une tumeur de vessie Sur 1343 patients hospitalisés dans le service pendant la période d’étude, soit 14 %.

Avec un sex Ratio : 1,23 pour P : 0,5049 nous avons obtenu :

L’âge moyen était de 53 Ans avec des extrêmes de [1 à 87 ans]. La tranche d’âge la plus touchée [41-60] avec 72 patients soit 39%.

Les patients résidaient : Bamako 79 (43%) ; Kayes 32 (17%) ; Ségou 25 (14%) ; Koulikoro 23 (12%) ; Mopti 13 (7%) ; Sikasso 06 (3%) ; RCI, Ghana, Mauritanie et Guinée 05 (3%) ; Toumbouctou et Gao 02 (1%).

La majorité des patients (135 soit 73%) avait été adressée par des confrères d’autres structures pour des raisons particulières. La profession la plus représentée était les femmes au foyer (menagères) 71 soit 38 % suivies des cultivateurs 56 soit 30 %.  Les analphabètes étaient au nombre de 133 soit 72 %.

L’Hématurie était la plainte principale (170 soit 92%). Le confort mictionnel était altéré chez ces patients.

Les examens d’imagérie, d’endoscopie et d’anatomo-cyto-pathologie ont permis de poser le diagnostic. La durée d’hospitalisation variait entre (06 Jours et 02 mois)

De decouverte tardive, le pronostic était sombre dans la plupart des cas. Nous deplorons 62 decès soit 34% de l’effectif durant l’hospitalisation.

Conclusion : Les tumeurs de vessie semblent plus fréquentes dans les zones d’endémie bilharzienne, de mauvais pronostic.

ABSTRACT :

Introduction: Bladder cancer is the second most common cancer of the urinary system after prostate cancer. It is much more common in men than in women.

The incidence of bladder cancer is on the rise in Mali, becoming a real public health problem.

• In men: 8.2 / 100,000 inhabitants and in women: 6.2 / 100,000 inhabitants according to the 2015 Mali cancer registry [15].

 Our work fits into this framework.

Patients and methods: retrospective and descriptive study collating 185 Patients hospitalized in the urology department of the CHU Point-G from January 1, 2015 to December 31, 2016, for bladder tumor. All these patients were selected for the study. The epidemiological aspects (frequency, age, sex, geographical location, risk factors), diagnostic, therapeutic and evolutionary were analyzed.

Results: 185 patients (102 men and 83 women) had a bladder tumor of 1343 patients hospitalized in the department during the study period, or 14%.

The patients resided: Bamako 79 (43%); Kayes 32 (17%); Segou 25 (14%); Koulikoro 23 (12%); Mopti 13 (7%); Sikasso 06 (3%); RCI, Ghana, Mauritania and Guinea 05 (3%); Toumbouctou and Gao 02 (1%).

The most represented occupation was housewives 71 (38% followed by farmers 56 or 30%). There were 133 illiterates, ie 72%. Hematuria was the main complaint (170 or 92%). The urinary comfort was impaired in these patients.Imaging, endoscopy and anatomo-cyto-pathology examinations allowed the diagnosis to be made. The duration of hospitalization varied between (06 Days and 02 months)Late discovery, the prognosis was dark in most cases. We deplore 62 deaths or 34% of the workforce during hospitalization.CONCLUSION: Bladder tumors appear more frequent in areas with endemic bilharzia, poor prognosis.Key words: Bladder tumor, epidemiology, hematuria, hospitalization.
